At a minimum, all resident individual care plans are reviewed every four months (S.I. No. 415 of 2013). By Implementing the requirements detailed in this procedure, the residential home ensures that all residents are reviewed on an ongoing basis and in line with their individual care plans.
This policy has been updated to incorporate National Screening Programme requirements.
Ownership of this document once purchased is strictly limited to one organisation, and licensed to that organisation only.
